New Patients
Insurance
We do accept and bill insurance for you. We participate with nearly all major plans, but please call to inquire about specifics. We also accept patients without insurance (on a fee-for-service basis).
Prescriptions
The state of Maryland allows “direct access” to physical therapy–meaning that you do not need a doctor’s order to seek PT. You may be required to get a prescription or primary care referral, however, if your insurance or diagnosis warrants it. We will review individual circumstances with you before the evaluation.
Parking Entry
Ample parking is available at 6 Tiger Way! Do be careful entering the lot, however – the corners are tight and you need to watch for other drivers leaving. An ADA ramp is available in the middle of the Antietam Ridge center.
General
Please arrive 15 minutes prior to your appointment for registration. Please remember to bring your insurance card(s), a photo ID, comfortable/loose clothing and appropriate shoes. If you have medication or other lists, we can copy them for you. Appointments are typically 45-60 min in length and include a mixture of hands-on treatment, movement / exercise instruction, and problem solving for your specific case.
Forms
You are welcome to download and complete the form below prior to your visit. If not, we will have it available for you when you arrive.
